What is this service?
The Community Falls Service provides triage, assessment and intervention to those aged over 65 who have had a fall. This service is provided alongside those offered by Stockport Foundation Trust and Age Concern Stockport to ensure an appropriate level of assessment for each individual.
This service includes:
- Falls Telephone Triage Assessment.
- Community Falls Clinic. The falls risk assessment in this clinic is provided by a Physiotherapist, Pharmacist and a Nurse. Each patient will also be offered a home hazard assessment on a separate appointment. Falls risk assessments can also be completed at home if appropriate to the individual.
- Balance Group. A 12 week exercise and education programme to improve strength, balance and awareness of how to reduce the risk of further falls. Following this programme participants will be offered another 3 months supervised exercise as part of the Falls Home Exercise Programme.
- Falls Home Exercise Programme.
A 12 month supervised and progressive home exercise programme to improve strength and balance.
- Advice and Education to staff.
The team are available to answer specific queries from members of staff across the health and social care teams and also provide training sessions to these teams.

Where and when is the service available?
The Community Falls Service team are based at the above address.
The different elements of the service are provided in different locations:
- Community Falls Clinic – Adswood Clinic, Wednesday and Thursday afternoons.
- Balance Group – Physiotherapy Gym at Kingsgate House, Wednesday mornings.
- Falls Home Exercise Programme –
This service is delivered in the patients own home.
Who is this service for?
The service is for those:
- Aged 65 or over
- Who have a Stockport GP
- Who have had a fall where either:
- there has been an injury,
- the patients walking or balance has not returned to their usual level following the fall
- or the patient has had one or more previous falls in the last 6 months.
The service is not for those:
- whose needs can be met by any other service
- who have (or had) medical symptoms related to their fall
- who cannot remember how they fell.
Before referral to the service any injury related to the fall or acute medical condition must be treated.
